I have a physician who codes all his post op visits with the same dx as the surgery ~ ie: 553.21 rather than V67.0

Is either way accurate or should he use both (and which order would you put them in?)

You would code the V67.09 and not the 553.21. The reason for the surgery no longer exists (assuming all went well that is), so there is no need to code that condition.
 
Thanks for the confirmation, Debra - it always helps to have back-up when explaining alternate ideas to the physicians.
