Indications: large perisplenic hematoma

Findings: large clots close to the spleen

Procedure details: Patient was taken to the operating room and placed supine on the operating table. Time out was performed and he received clindamycin 900 mg IV 30 minutes prior to incision. He was then given IV sedation. His left chest and upper abdomen was then prepped and draped in a standard surgical fashion. A previous mark was seen from the US guided location of the hematoma. Marcaine 0.25% was then injected into the skin around the 11th intercostal space. Bovie was then used to make an incision parallel to the 11th and 12th rib. The fascia was then taken down and the intercostal muscles were retracted. The peritoneum was then cut with the Metzenbaum scissors and the hematoma cavity was entered. The hematoma clots were slowly evacuated until no further clots were visualized. The cavity was then irrigated with warm normal saline. A 19 French JP drain was then placed into the cavity. The fascia was then closed with a running 3-0 vicryl. The skin was reapproximated using a running 2-0 nylon suture. The drain was then secured with a 2-0 nylon as well. Sterile dressing was placed over the incision site. Patient was then awoken from anesthesia and transferred to pacu in a stable condition.
