Wiki PRC with capsular interposition flap

Hello, would this be best reported as CPT 25332-Arthroplasty, wrist, with or without interposition, with or without external or internal fixation ? As, CPT 25215-Carpectomy; all bones of proximal row has a CCI conflict with 25332. I appreciate any and all feedback!

I wanted to mention that CPT 25215 for the PRC was requested as well as 25447 for the capsular interposition flap. However, I am thinking for this specific case that only 25332 would be reported.

First of all you should be coding 64772 for excision of the PIN.

For PRC with capsular interposition flap, you can EITHER code 25332 alone (PRC is integral to the arthroplasty in this case and 25332 is considered a more extensive procedure)

OR

Code 25215, 25447. Slightly higher RVU I think.

Both approaches have been considered appropriate by ASSH in the past.
