I currently work at a mental health / addictive disease crisis stabilization unit which is an inpatient facility that allow clients to stay, on average, 7-10 days. We sometimes have individuals come in for treatment and when they do, they see the RN Nurse first, for a Nursing Assessment. However, I have seen where the client will change their mind and leave after this assessment without ever having seen the doctor who makes the diagnosis. What billable diagnosis code can be used by an RN so we can get paid? We have tried the Z03.89 but the claims fail.
