Patient is scheduled for a Pre Op Cardio Clearance with a echo and office visit. ex: 99214, 93000 with diagnosis V782.81 and 414.00

Can ICD-9 code V72.81 be billed as a primary diagnosis?

it is best practice to list the 2ndary diagnosis code the reason for their surgery.
Medicare actually prefers the V code "cardio cx clearance" secondary and the reason for the surgery primary... there is an article re: this on the website:www.wpsmedicare.com
