General surgeon did full exam for colonoscopy screening but never mentions in the exam that it is a pre op. Can I code pre op exam?

According medicare and several third-party payers, if the physician is performing a screening colonoscopy, they will not pay for an E/M visit separately. They consider it bundled. Their rationale is that if this a screening meaning the patient is not showing any symptoms to warrant a diagnostic colonoscopy, there is no medical necessity to see the patient before hand.
