Pre-operative Exams

our gastroenterologist get several referrals from VA for colonoscopies - when the patient comes with no symptoms and just for pre-op for colonscopy we do not charge for the visit and it is bundled into the procedure. The problem comes when the carrier does not pay for consults and during the pre-operative exam it is determined not to do the procedure. What ICD-9 code(s) / CPT do you charge for payment? Can you just report the V72.85 peroperative exam code to get payment or do you need additional documentation from the referring physican to show medical necessity? Thanks
