I thank you before hand for the reply. If the patient has anemia in the problem list with ferous sulfate on med. do we still treat anemia as complicating the pregnancy?
What does the provider mean when he/she states S= D under Assessment

Hi, S=D refers to size = dates, so the fetus is measuring on track for weeks of gestation. Per the ICD10 guidelines, it's up to the provider to document that something is NOT affecting the pregnancy, so in absence of that documentation, you would code it.
