We have many physicians who use dx codes that are inappropriate as the primary code. For example, they will use Z3A.12 (12 weeks gestation of pregnancy). Is it enhancing or applying a dx on your own if you use Z3482 (Encounter for supervision of other normal pregnancy) when that code is not listed as the primary and use Z3A.12 as the secondary ?
