Dx code is V72.83, can I leave it as is or do I need the reason for preop to go with it. The provider did not say what the Pre Op physical was for.

The reason for the preop is not required, just use the V72.83.
 
According to the Official ICD-9-CM Guidelines for Coding and Reporting, Section IV. Diagnostic Coding and Reporting Guidelines for Outpatient Services " Patients receiving preoperative evaluations only - For patients receiving preoperatice evaluations only, sequence first a code from category V72.8, Other specified examinations, to describe the pre-op consultation. Assign a code for the condition to describe the reason for the surgery as an additional diagnosis. Code also any finding related to the pre-op evaluation."

The guidelines can be found in the front of your ICD book.
