My institution is using Z01.818 for pre-endoscopy encounter since a visit for screening colonoscopy (as and example), unless is high risk, is not payable. The question is, can Z01.818 be use as a first code when there is no risk prior to endoscopic procedure?

Might depend on the documentation. To me, I think that if they're just looking for a code to get something paid, then, no. But if they're really evaluating and doing an exam to make sure the patient is ok for the procedure, whether there's a risk or not, then yes.

Could you possible use the reason for the procedure as a primary dx and then supplement with the other codes you had mentioned?
