Ok here is the dilemma; patient seen for a prevent and a problem visit, patient also receives immunizations. Would you put a 25 modifier on both the prevent & problem visit?

Do you know of a reputable site that will answer this in black-n-white?

25 mod on the preventive code since the E&M was the additional service, not too sure of a reputable site. I'm always right so you can take my word for it. At least that's what my momma always told me. JOKES!
