I have claims that are denying when the provider is coding a preventative wellness exam for a particular managed care plan with the primary diagnosis of V72.31. I have just started to see this. Should it be being billed with the V70.0 as primary and then the V72.31?
 
MCO's, don't you love 'em? Ha.

What MC plan? V72.31 is a gynecological examination. Is that the primary reason the patient was seen or is the V70.0 applicable to the visit?
