Hello - my office has recently been running into issues when billing E/M codes 99384-86 and 99394-96 for annual GYN exams to private insurance companies - specifically BCBS, POMCO and AETNA. They are denying stating that those codes mean the patient was seen for their annual wellness exam/physical with primary doctor. We have used GYN related diagnosis codes to try and clarify: Z01.419 pelvic exam for example. I get nowhere trying to dispute as they say it is processed as an annual wellness exam. Even if we are to bill under a non-annual office visit E/M the companies are then applying deductible costs which shouldn't normally apply for an annual exam. Does anybody have any guidance on this?
