Does anyone know if there is a Preventative Physical Audit tool out there and if so where we can find one?

My office is having a terrible issue with documentation on Physicals, and since CPT is so vague on what exactly is required (not black and white), would really like an audit tool to use when reviewing for these service.

Any info would be great, thanks in advance!
