During a preventative visit a Doctor is recommending to take Ibuprofen for leg pain, that the patient is experiencing would this be a low risk or a moderate risk? Would you bill an additional E/M with this scenario during a preventative visit?
 
It depends on the documentation. Is there enough if you take out everything that was related to the preventive visit to bill an E/M and support a 25 modifier?
