Wiki Preventitive Evaluation and Management

I am looking for an extensive description of the Preventitive Evaluation and Management codes and what all is covered in that code. (example. review of chronic illness diagnosis, contraception, etc) Where does the Preventitive Evaluation and Management visit stop and an additional office visit start if other medical conditions are addressed in the visit? If anyone knows where to find a more extensive description of these codes than the CPT Book has, I would be interested in getting a copy of that.

This should give you a good description - and definitely check out the various forms and articles they have on the AAFP website. I have found it to be an amazing source of information. They have tons of practice management tools and coding info.
