When a physician manages minor problem like sore throat, Constipation with OTC along with preventive care services, can we code both E/M with modifier 25 and preventive care services.
 
If the provider completed a preventive visit and also performed a separately identifiable E/M for the complaints, then yes, you could bill for both. The documentation would have to clearly differentiate what was done during each service.
