I have a pt who has hypertension and diabetes who came to our clinic for an a preventive H & P. The most of the exam was completed on the first day. I coded 99396 with the Dx of 250.00 and v70.0. The provider completed a baseline EKG and immunizations on the seconds vist. My questions: What diagnosis code will work for the baseline EKG ? Do I put the physical code on both days? Looking for some help. Thanks
