My question centers around code 99404 for preventive medicine counseling. This is a time based code that is approximately 60 mins of prevention counseling. What happens if the provider spends 90 mins in prevention counseling? Since code 99404 is only for 60 mins, how do you report the additional 30 mins of counseling? Would you use a prolonged services code (99354) in addition to 99404?
 
Code 99354 states use 99354 in conjunction with 99201-99215, 99241-99245, 99324-99337, 99341-99350, 90809, 90815. None of the preventive codes are listed. So you would not use this code with 99404.
