I am not sure how and if we can bill all 3 codes. Patient has a preventive service 99396, and a problem visit 99212-25, AND Tobacco Cessation 5 minutes 99406 with additional documentation about quiting, products to assist in quiting etc. in the chart note. Can all 3 be billed? For example would the 99396-25 dx V70.0, 99212-25 dx 692.9, and 99406 dx 496 & 305.1 all be billed? I have never seen all these billed before but now our physicians are questoning if they can. I have seen 99396-25 with 99406, OR 99212-25 with 99406 but not all 3????? Any help would be greatly appreciated!:confused:
 
my first questions is what did the patient come in for preventive or exam? I wouldn't use a -25 on both E/M. You can bill all three together but you may be better off combining them and billing a high level E/M.
 
Patient comes in for a preventive exam but once the patient is being seen by the doctor patient also discuss a problem and the doctor performs a seperate significant E&M sevice besides the preventive visit and also treats the problem visit.
