Wiki preventive visit and modifier 25 help

When a patient is coming in for preventive visit and is also getting a flu shot or other vaccine. Would you add a 25 modifier to the preventive code? Or if they are having physical and other services hearing,vision test would you use the 25 modifier? Thank you.
ps. If they had the physical and vision,hearing tests would you add a 59 modifier on one of the vision or hearing test?

Yes, you would add the modifier to the preventive visit or physical visit.
On flu shot, add your med and then the admin. code.
On hearing, vision, etc. Phys code modifier 25, then vision code no modifier, then hearing test, modifier 59
