Does anyone know of a good preventive visit / exam checklist or template for non-Medicare patients?

Preventive Exam

Is it for adults or children'? . If it pertains to a child checkup, Texas Medicaid has a template/checklist on their website under preventive checkup you can use.

You could also check with your insurance carrier websites, some of them do have their "own" template.

What I have done is gathered these from the various carriers to compare, I found that the majority of them are the same. You could then just create one for your organization to use.

Just an idea.
