If patient comes in for annual exam and also gets immunizations would you use modifier 25 on the preventive visit?

Ex: 99396
90734
90471

99396-25 v70.0
90737 v03.9
90471 v03.9

Or do you use the mod. 25 just on E/M codes for injections and immunizations
 
hi kimb - yes, I use a .25 on the preventive when an immunization is given. As long as documentation supports the charges ;) (The preventive codes are "E/M" codes as well as the "level" codes and consult codes too)
