I am looking for a rate for CPT code 99401 (Preventative Medicine & Counseling; approximately 15 minutes, includes: waist circumference, body mass index and blood pressure). The code is not covered by Medicaid or Medicare; so I wanted to determine a reasonable rate. Also determine if the code has been replaced or if it requires a routine Diagnosis code for reimbursement.
