Provider documents a primary diagnosis of flat feet, insurance is billed for OV and Xrays. The claim is denied stating non covered diagnosis. Contracture of Muscle and Pain were also documented. Can we change the primary diagnosis and rebill? If so does the doctor change the note or just write an addendum to note changes?

If the diagnosis you want to now use is already documented, there is no need to change the note.
If the diagnosis you want to now use was not already documented, it would look mighty suspicious to have the addendum.
By diagnosis already documented, I mean the words/description/context. Not the code. The code does not need to be anywhere on the note, only on your claim. It seems like in this situation, it might be appropriate to send a corrected claim since it is already documented and your original claim incorrectly did not list the diagnoses.
