Can I please get some help understanding primary diagnosis codes on hospital subsequent encounters? If the patient is admitted with a diagnosis of respiratory failure (ex) and on next day the hospitalist documents that the patient developed a fever, should the primary diagnosis on the subsequent encounter then change to fever followed by respiratory failure as a secondary diagnosis or should the primary diagnosis remain as respiratory failure as it was the reason for admittance.
