Primary procedure lysis of adhesions secondary finding hernia.

The physicians intent for the procedure was to do lysis of adhesions, but when he performed the adhesions takedown he noticed the patient had a reducible incisional hernia. Which code do you bill the lysis of adhesions carries a higher wrvu and was the original intent of the surgery? However it is bundled with doing hernia repair and lysis of adhesions is usually part of all abdominal surgeries and bundled.
 
Make sure the op-note supports the lysis of adhesions as primary and that hernia is incidental finding. Then you can bill for the Adhesions and incidental hernia would then not be reported. If its documented the other way around bill for the hernia and not the incidental lysis.
