Prior Authorizations - Bilateral Injections

Looking for guidance on some denials we are encountering with Anthem of Indiana and bilateral injections. Prior authorizations are being requested and approved with the correct units and diagnosis for bilateral injections. When we submit the claims for the bilateral injections with the approved bilateral authorization, they are under-reimbursing, stating that we only requested one dose. Has anyone else encountered this, or have a copy of the UM policy on how many doses need to be requested?
