If the visit is for non-threatening DX'S (back pain,conjunctivitis,stiff neck,dermatitis) but NEW to the physician can I count for example;
The patient CC is BACK PAIN = as a SELF-LIMITED which is ONE POINT
OR
=as a NEW PROBLEM to the examiner which is THREE POINTS

Well, that depends.

If I have back pain because I raked leaves for the first time this season and haven't been exercising at all, it's probably self-limited (i.e. it will get better by itself with rest).

On the other hand if I have back pain or a stiff neck because I have a ruptured disc ... that will not go away by itself.

Your scenario as it reads to me would count as a new problem to the physician and credited thusly. You can err on the side of caution if you wish, but, you cannot assume from that note that it is self-limiting. 3 points! Thanks for listening. Anita
 
No, Anita, the logic seems to be at err. I am 58yrs old and I am not able to move/bend my right thumb since 2days properly and New to the Physician. So you would give 3points ?
