Wiki Procedure-to-Diagnosis Relationship Report

I have been unable to find the Procedure-to-Diagnosis Relationship Report on the CMS Website. I need to find out the correct DX for Hepatitis A and B vaccines and for Hepatitis A, B, and C lab tests. I would appreciate any help.

The diagnosis code for vaccination of all three is V05.3 (they are all viral hepatitis).

The diagnosis code for laboratory screening for all three conditions is V73.89. I'm not familiar with the report you mention. However, it sounds like an NCD or LCD.
