Looking for some clarification when billing add on cpt code 93623 in a hospital/facility based setting, place of service 21 or 22; is it appropriate to also bill the drug code on a cms 1500 from? (J3490 injectable form of Isuprel). My thought is the drug would be supplied by the facility and included in their billing. :confused:
