I have 90837 that our LCSW billed with 99354 because patient was seen for 120 minutes. Payer has denied the 99354 needs primary service.. Suggestions on how to appeal this?

You need to access the CPT2013 errata . In there it shows that 90837 was added to the allowed first listed codes for 99354, then appeal the denial with that info
