I am currently discussing this scenario with one of our physicians. He performed an EGD, snared the PEG, and cut and removed then placed a button. No new incision was made. My office manager instructed me to bill:
43247 - EGD, flexible, transoral; with removal of foreign bodies and
43762 - Replacement of gastrostomy tube, percutaneous, includes removal, when performed; without imaging or endoscopic guidance; not requiring revision of gastrostomy tract.

However, these codes are contradictory, as 43247 is for endoscopy and 43762 states without endoscopic guidance.

Does anyone have any insight on this?
