We are provider based billing and our family practice physician does a urine dip in the office. Can the hospital bill for the urine dip since we are provider based?

Yes, the hospital would bill for the lab test on the UB claim; there would not be a physician bill for that. There is no separate payment for a urine dip if billed with an office visit - it is packaged into the APC rate - but it should be billed anyway as it is counted toward the rate calculations and the hospital's utilization and cost reporting.
