Does anyone know where I would locate info regarding physician treating a family member? Specifically, Anthem policy. I can only locate info related to Medicare. I have a physician who has surgically treated his son due to sports injury in an urgent situation.
 
It states this exclusion on page 55 of the Provider manual:

Family Members Excluded
REMINDER: Any services rendered by a provider to a family member such as a spouse, child, stepchild, parent,
stepparent or grandparent, etc. are excluded as services you may bill to Blue Cross.
