Hi,
I will be trying to help a psychologist with his documentation and wanted to know if anyone can share examples of good documentation for codes 90791, 90837, 96101, 96103, 96150, 96151, 96152.
Also, for codes that state "face to face time spent with patients" (i.e. 90837, 96101, 96150...) are you all requiring providers to document in their notes that the patient was in the clinic/office?
