I am looking for a good psychiatric services audit sheet for outpatient services. Something that would cover documentation elements for psychotherapy and pharmacological management services. Thank you for anyone's input. I have a Coding and Billing for Behavioral Health 2008 but it doesn't have good documentation or audit tools.
 
Psych Audit

I perform 100% chart review at our facility, so I guess yes, I perform audits. I work for a federally qualified health center, we perform out patient services. Did you need some information?

You can find an audit sheet for Pysch outpatient visits at E&M University. com. It is a website for physicians to educate them on E&M coding but you can get a copy of the audit sheet there.
