I have a psychiatrist who wants to bill a 99212 for followup visits. Does he have to do a physical exam to do this? Thanks so much
 
A 99212 is for an established patient. An established patient requires 2 out of 3 key components - history, exam, and medical decision making. If he doesn't do an exam, the documentation for history and medical decision making must be present and meet the requirements for 99212.
