I bill Physical Therapy, Occupational Therapy and Speech Therapy. We have always bill PT, OT and ST on separate claim forms. Does anyone know if these services can be billed all on the same UB? We are an outpatient rehab facility so we bill on a UB.
 
I've often seen them billed on the same form. I don't know why you wouldn't be able to, unless one of your payer contracts specifically requires it be done this way.

Here is a tip on billing PT and OT....put referring provider on claim. And add modifer GO, GN and GP per type of provider giving the therapy. I hope this tip helps you too.

Yes, I have seen them on the same UB. Agree with Thomas, there may be payer policies that say they should be separated.
