pt discharged but stuck in hospital

My doctor is discharging pt say on friday, but the pt stays in the hospital until monday because there is no bed at the nursing home. When he does his report he says" pt discharged friday, but was in hospital waiting on bed until
monday". Is this the correct format? The hospital is showing discharge as
monday to nursing home care. Help!:eek:
 
He is only billing to friday, but the hospital will of course bill until the bed in the transfer facility is available. But how should my doctor document this?
 
There must be a way to document this as it happens all the time. Even my husband several years ago was discharged and had to wait for a bed to open at the transfered to hospital for 3 days
