Wiki PTCA and CABG Dx coding - Hope you can help me

Hope you can help me with this note's Dx codes. Here goes:

He has an extensive cardiac history including CAD w/PTCA 5 years ago and CABG 2 years ago; HTN, MI 3 years ago, CHF, and A-Fib. He is currently on Lasix, Digoxin, Coumadin, Hydrocholorothiazide, and Nitroglcycerin. Work up revealed, Dx: Acute CHF.

Also, how would you sequence the Dx codes?

All the others appear to be history of. Is the only dx Acute CHF? IF so, 428.0 since he is not indicating if Diastolic or Systolic.
