Is it permissible to include diagnosis codes from the body of the report i.e. ROS or HPI, that the physician may not have included in the impression? e.g. physician states knee pain, shortness of breath, fatigue in the ROS but no mention of it in the impression. I was informed to only code what is in the impression as well as the sequential order.
