I have had multiple charts lately that say my dr admitted the pt on 1 day but his H&P is dated 1 or 2 days later example pt admitted 12/1 but the H&P is dated 12/2..this is new to me I am use to coding for ER's now I am coding for Pulmonologists, so thank you all for any and all help..alice
 
I'm assuming you're coding professional charges.

Make sure the date of the note is the actual date of service (sometimes transcriptionists will place the date of dictation as the DOS...you just need to verify with your provider).

Once you've established the DOS, that's what you bill....the date of the actual face to face visit. The code you report is not specifically for "admission", it's for initial inpatient hospital visit, whenever that may be. That date doesn't need to match the facility's actual admission date.
