Hello Friends,
can someone help me to find out Pdx.

Pt came to hospital with chief complaint of sob. Pt denies any leg pain and edema or swelling. Radiology reports confirmed both PE and DVT of lower extremity.
Physician documented only PE in H&P. Remaining all notes impression stated both PE and DVT and treated with IVC filter. What will be the PDX if DVT is had higher relative weight age than PE.
 
Depending on hospital Coding Guidelines. Most indicate that you cant code from Radiology reports. I would use PE as PDX if indicated in H&P or PDX that is used on OP Report if there is one. Hope this helps. :)
