According to CPT codes 94010-94799 include laboratory procedure(s) and interpretation of test results. Does anyone know if the interpretation has to be on a separate form or can that interpretation be within the body of the office notes or withing the test itself? Thanks alot
 
I believe as long as an interpretation is documented by the provider, whether in the office note itself, on the test form or on a separate report, either location is acceptable as long as it clearly reports the findings. Simply initialing the report is not considered "interpretation and report".
