I was wondering if you could help with some questions?
1. How to code cryoprobe for excision? Ex a foreign body removal of a peanut—the cryo is used to freeze and the peanut attaches to the tool, and it is removed – can we use 31641 or 31635 or BOTH?
2. Plastic bronchitis – there are a few cases a week – how do we code the removal? 31635 OR 31641 OR BOTH?
3. Pulmonary has heard other groups are billing for multiple units 31647/ 31651 for placing multiple valves—why does the ama not state per valve- instead they state per lobe?
4. DISE billing – CPT 42975 can this be billed with a patient that has a tracheostomy? If they are looking to potentially see if the pt “could have” OSA? Can we confirm if we should just use for OSA patients vs patients with cross codes in encoder.
5.In some notes providers mention a CT scan showing a lesion. Then we mentioned the use of CT navigation software to navigate to the target. In the attestation we asked for a 31627 (CT navigation). Is there something different we should be listing to help make sure this gets billed for? This is something that often takes a considerable amount of time/effort for the pulmonologist to set up for the scope and greatly impacts the care in the cases that we do use it.
