Originally billed Medicare Q4131 1 unit. The correct units the patient received was 5. Reopened claim with Medicare for the correct total until 5. Received payment for additional 1 units. Medicare stated they cannot reopen claim. They told me to bill a new claim with 3 units and a modifier to indicate it was not a duplicate. I cannot find a modifier for Q4131 regarding duplicate. Any suggestions??
