Wiki QS and QZ modifier billed for Medicaid claim

I have a bunch of denials for Medicaid for using both of the modifiers on a claim. Does anyone have any suggestions? I found sites that say to use U1 or U2 but they are for physicians directing the CRNA not for CRNA solo
 
Please refer to manual. If it is CMS1500
You have to use modifier 99 for multiple modifier
In box 19
99= QZ+QS ( QZ=CRNA, QS= Monitor Anesthesia Service) 1120-1320 SEE ATTACHED
Below Box 24
1120-1320
Bill hard copy with Anesthesia report indicating start and stop time.
