Does anyone's providers include qualifier diagnosis and ICD10 codes as part of their documentation and do you code from them? At the end of the dictated report the provider is asked a series of questions about the patient's diagnosis and led to a diagnosis with the attached ICD10 diagnosis code, separate from their final impression. The document is then signed by the provider. Very often this qualifier is more specific than the diagnosis dictated in the provider's own words.
